Booking an Appointment
PHONE CONSULTATIONS: If you would like to book a free 10-minute phone consultation to discuss your treatment needs/goals prior to scheduling a new-patient evaluation, please follow the scheduling link below and select the corresponding option. I will reach out to you at your scheduled consultation time. If you prefer to schedule an initial evaluation without completing a phone consultation, please follow the instructions for new patients in the next paragraph.
NEW PATIENTS: When you are ready to schedule your first appointment, please click the scheduling link below and select the Initial Evaluation option at your preferred time. Upon requesting a new-patient appointment, you will receive an email with instructions for completing an intake packet, which includes a health history questionnaire and treatment consent forms. Your appointment will be confirmed only when you have submitted your packet. If your intake packet is not completed quickly and submitted during normal clinic hours you may lose your requested time slot. I have this policy in place so that I can perform a preliminary benefits verification. Please read the information in the next section for details about insurance coverage.
EXISTING PATIENTS: Please use either the appointment scheduling or patient portal link below to schedule a follow up appointment. If you have not yet set up your portal account, you can do so by clicking the portal link below and selecting “Don’t have an account”. Your portal account can be used to manage your appointments, view prescriptions, pay invoices, and send me secure messages. Appointment requests for existing patients will be confirmed automatically.
Insurance Coverage & Payment Information:
Valid Payment Method: All patients are required to provide a valid credit card (not an HSA/FSA card) to establish and maintain care at Chrysalis Mental Health. If you are a new patient and want to use an HSA/FSA card as your primary payment method, please enter your card information on the HSA/FSA consent form in your intake packet or provide your card details during your first appointment. Established patients may add their HSA/FSA card as their preferred payment method via the patient portal or provide me with their card details during an appointment. Any patient using an HSA/FSA card as their primary payment method must maintain a second credit card on file for fees not covered by their HSA/FSA account.
Any card information that you provide will be stored in an encrypted HIPAA-compliant platform; secure card details are never accessible to me, my billing company, or anyone else.
Insurance Benefits: If you are a new patient, I will make every effort to determine whether your services will be covered by insurance before we meet. However, my benefits verifications are not guarantees of coverage. Even if I am contracted with your general insurance carrier (e.g., BCBS, Aetna, Moda, Providence, etc.), you may have a unique sub-policy that is not in network. It is my goal to catch this before you incur unexpected costs, but only your insurance company can guarantee your coverage. This also applies to patients whose insurance policy changes after they establish care with me.
Visit Type and Format: Initial evaluations are required for all new patients, even if they are referred by another provider.
Evaluations for new and existing patients will be automatically scheduled via telehealth. If we have agreed to an in-person appointment via email, portal message, or during an appointment, I will manually change your visit format after you have scheduled.
