Chrysalis Mental Health LLC
Visit Format & Telehealth Information
Chrysalis Mental Health offers “hybrid” options for patient visits. This means that patients may be seen in person, via telehealth, or a combination of both. Many Chrysalis Mental Health patients prefer to meet via telehealth for ease of treatment access, although in-person visits are also available for those who need/prefer them. Patients who have previously been seen exclusively via telehealth may request to be seen in person at any time during their care.
Because Chrysalis Mental Health serves patients throughout the state of Oregon, meeting in person can present challenges for individuals located outside of the Portland metropolitan area. While some patients are happy to travel intermittently for in-person visits, others may find the physical distance of our clinic to be unsuitable for their needs. When this occurs, Chrysalis Mental Health will provide patients with a list of provider referrals near their place of residence. Referral lists are customized depending on the specific patient’s condition(s), preferences, and/or other unique needs.
Additionally, some patients require in-person visits for safety reasons, whether regularly or intermittently. For example, patients who prefer to meet via telehealth but who are taking medications that affect blood pressure/heart rate (e.g., stimulants, alpha agonists/antagonists, and beta blockers) or drugs that have the propensity to cause abnormal movements (e.g., antipsychotic medications) or other side effects will be seen in person intermittently to evaluate for issues that cannot be observed remotely. Please note that these examples are not exhaustive. Prospective patients will be educated on visit format options and/or possible in-person requirements during a phone call prior to attending their initial evaluation, and/or at any point thereafter should their treatment plan or legal/insurance requirements related to telehealth services change.
For patients who prefer to meet with their provider primarily via telehealth, Chrysalis Mental Health utilizes a secure/HIPAA compliant video platform. Patients seen via telehealth are also advised of their provider’s credentials, physical location, limitations of care provided remotely, and their provider’s alternative contact information (e.g., secure phone number, email address, and/or messaging via their EHR portal account) that can be accessed in the event of poor telehealth connectivity. Furthermore, patients are advised that their provider will reach out to them by phone should their telehealth connection fail at any point during an appointment such that no action is required on their end.
When telehealth services are conducted, patients will also be asked to confirm their physical location at the beginning of each visit to ensure they are located in the state of Oregon where the provider is licensed to provide care. This requirement is also outlined in Chrysalis Mental Health’s intake/consent paperwork. If a patient is located outside of Oregon due to travel and is experiencing acute symptoms or side effects that require in-person attention, their provider will direct them to their nearest urgent care or emergency medical facility depending on the severity of their concern(s). If the patient requests or requires collateral information to facilitate a safe transfer of care, their provider will contact the referred clinic/facility to debrief the care team on information pertinent to their condition(s). This plan also applies to patients who cannot easily access in-person visits in an emergency due to their distance from Portland.
