Insurance & Payment

I am in network with Blue Cross Blue Shield (Regence, Anthem), Moda, and Aetna. I will be fully contracted with Pacific Source on 3/1/25 and am currently pre-booking patients with Pacific Source health plans.

Please note that I am unable to see patients who have Medicare and/or Medicaid (Oregon Health Plan, Apple Care, Molina).

If I am out of network with your plan, your insurance company may still be willing to pay for my services. In such cases, you will be asked to pay at the time of service and I will provide you with a “Super Bill” (itemized receipt) that you can submit to your insurance company for reimbursement. Each plan varies with regard to their coverage of out-of-network services.

**Please call your insurance company to verify your benefits, co-pay/co-insurance, and deductible prior to scheduling your appointment as I am unable to guarantee your coverage. Clients are required to provide a valid payment method to book an appointment. This will be used for copays/coinsurance and fees not covered by insurance.
